An unshaven hair transplant is performed without cutting the donor-area hair, other than trimming a narrow strip (a few millimetres wide) where extraction takes place — a strip that's then concealed by the surrounding long hair. Existing hair in the recipient area is left uncut as well. The goal is for the patient to return to daily life and work without a visible sign of the procedure to those around them.

This approach adds complexity to the procedure: working among uncut, long hair takes more time and care than working with short hair. As a result, the number of grafts that can be taken in a single session with the unshaven approach is generally more limited than with a fully shaved FUE session. The method suits limited regional thickening or closing moderate loss in the hairline/crown area better than it suits very extensive full-area loss, where reaching adequate density in one session becomes harder.

An unshaven transplant is chosen by patients who, for work or social reasons, don't want a shave line visible and need to return to a normal appearance quickly. Hair needs to be at least 4–5 cm long for this method; if hair is too short, the donor strip can't be concealed. Suitability is determined by weighing current hair length against the extent of loss together.

  • Patients whose donor density is already sparse, making the strip hard to conceal even with long hair
  • Time-constrained patients needing a high graft count reached quickly, for whom a longer session isn't practical

How Is It Performed?

  1. Marking the donor strip — A narrow strip a few millimetres wide, where extraction will take place, is marked; the surrounding long hair is preserved.
  2. Local anaesthesia — The donor and recipient areas are numbed.
  3. Extraction within the narrow strip — The punch works only within the marked strip; the surrounding long hair is left uncut.
  4. Graft sorting and holding — Follicles are counted and stored in a suitable solution.
  5. Unshaven channel opening — Channels are opened in the recipient area among existing long hair strands, without cutting them.
  6. Graft placement — Grafts are placed into the channels at an angle matched to the existing hair's direction.
  7. Wash and concealment guidance — Initial cleaning is done, and the patient is briefed on how to cover the donor strip with their long hair.
